Initial Comments:
A merger (effective February 28, 2008) and legal name change (effective May 29, 2008) was received on August 24, 2020. The federal tax identification information/number which remained was that of Carson Valley School. The legal name changed to Carson Valley Children's Aid. The facility remains in compliance with the requirements of 42 CFR Part 483, Subpart G Requirement for Psychiatric Residential Treatment Facilities.
